COVID-19 Update

Death & Birth Certificates Requests, effective December 1

Due to the increase in COVID, All Birth and Death Certificates will be mailed to the individual making the request, you will have the option to have them sent by USPS or Certified Mail.  There will be no option to pick them up in person.

This will remain in effect until further notice.

 Death Certificates

Effective December 1, 2020: Due to the increase in cases, and health and safety concerns, we have decided that there will be no in-person pick up option available for death certificates, effective immediately. This guideline will be in place until further notice. We are sorry for the inconvenience, but the safety of our community during Covid-19 pandemic is of importance to us.

All application and payment needs to be submitted online, death certificates will be mailed once we receive the payment. If you do not have online payment option, please mail the payment to us. Death certificates will only be mailed out once the online application is submitted AND the payment received.

 This will remain in effect until further notice.

Vital Records (Births & Deaths)

All births and deaths within Fishers city limits will have records available through the Fishers Health Department. 

Please note there is a $10 shipping cost (in addition to birth certificate fee). No walk in's are allowed. Learn how to apply for vital record here.

Request a Birth Certificate. For all birth and death records outside of Fishers or for births/deaths prior to April 24, 2020, you may contact the Hamilton County or the Indiana State Department of Health (ISDH). For questions, contact ISDH Vital Records at 317-233-2700.

Burial Transit Permits
