Death & Birth Certificates Requests

Our office is responsible for providing vital records pertaining to births and deaths in Fishers, Indiana, from April 24, 2020, to present. Vital records prior to this date may be requested from the Indiana Department of Health Vital Records office by calling 317-233-2700 or the local health department in the county where the birth/death occurred.

Birth and death certificates serve as official documents printed on certified security paper and used for legal purposes to prove a life event. 

Death Certificate requests can be made online 24/7 or in person Monday through Friday between the hours of 9:00 am and 3:00 pm. At present, all birth certificate requests must be made online.

You may come to our office to complete a death certificate request or request a death certificate online. 

Requests can be made in person between 9:00 am and 3:00 pm Monday through Friday. Most requests will be fulfilled the same day. Please click here to verify eligibility for who is legally allowed to receive a death certificate and for documentation needed to complete requests. Where Can I Pick Up?

Appointments to pick up records requested online are available Monday - Friday between 9 a.m. and 4:15 p.m. Please contact the Vital Records Registrar by email or call 317-526-7102 to schedule your pick-up time.

The Vital Records office is located at the Customer Service Building at 3 Municipal Drive, Fishers, IN 46038.

Fee Schedule

Fees are in accordance with chapter 180 of the City of Fishers Code of Ordinances. Please note there is a $10 shipping cost for all mail sent by Certified Mail. All records sent by USPS are free.
